Telehealth consent

Telehealth programs offered through Platform Health's store are provided by an independent medical group. The medical group is not owned or controlled by Platform Health or Platform Health and makes all diagnosis, treatment, and prescribing decisions.

The medical group's own Telehealth Informed Consent is presented to you in its visit flow, before any visit. You read and sign it there. Platform Health stores a copy of your signed consent for the medical group, with the date and the version you signed.

Before a visit you will also see where you need to be located for the visit, the medical group's name, and its Notice of Privacy Practices.

Medical evaluation required. Treatment and prescription approval are not guaranteed and are determined solely by an independent licensed clinician based on medical appropriateness.
